Staff must follow the following steps for all field trips:
*AHS & AMS Staff: Follow the AHS Field Trip Checklist prior to submitting the Field Trip Request Form.
- Staff submit Field Trip Request Form to Building Administrator
- Once approved by Building Administrator, submit transportation request if necessary
- Submit approved Field Trip Request Form & Student Roster to Nurse
- Submit a Sack Lunch Request, if needed
- Submit Overnight/ Out of State Travel Requests to School Board if necessary (at least 30 days in advance of trip)
- Provide parents/ guardians with Field Trip Student Permission Slip
