Sports Fees
There is a $45 fee to participate in a sport ($180 maximum per family). Fees may be paid online or in the main AMS office. To make an online payment or pay fines/fees with a credit card, click HERE. (Login information is the same as your Skyward account.)
If your student is eligible for Free & Reduced lunch your sports fees can also be waived. For more information, visit the Food and Nutrition page or contact Juanita Maltsberger at (360) 503-1377 or jmaltsberger@asd103.org.
Final Forms
Athletes/parents/guardians must complete all required forms in FinalForms to participate on the first day of practice. (Login information is the same as your Skyward account.)
Sports Physical
A sports physical is required to participate in athletics** and must be completed/turned in PRIOR to the first practice/tryouts. Physicals are good for 2 years. The completed physical form can be uploaded directly into FinalForms or returned to the AMS front office. You can download a copy of the required Sports Physical Form here or you can pick a paper copy up in the AMS front office any time.
If you are having trouble scheduling an appointment with your current health provider, the Island Hospital Walk-In Clinic on M Ave does same-day sports physicals. They are open 7 days a week, 7am-7pm (9am-5pm on Sundays). They recommend NOT visiting on Sundays as that's typically a very busy day and checking to see how full the parking lot is before coming in. They are a walk-in clinic and as such, there may be a lengthy wait if they are busy at the time you arrive.
Insurance
To protect all prospective student athletes, the State of Washington has mandated (per RCW 21A.400.350) that all participants be enrolled in a current insurance program and/or participate in the District's accidental medical insurance program.
When completing paperwork for your child, you will be required to provide either proof of your own insurance, or enroll in the District's accidental medial insurance program. To enroll to the district's current provider here Myers, Stevens, and Toohey.
Insurance Waiver/ Exemption
No student will be denied the ability to participate solely because the student's family is unable to pay for insurance (Board Policy 2151). The superintendent or designee may approve partial or full waiver of premiums to permit all students to obtain the required medical insurance.
