Staff must follow the following steps for all field trips: 

*AHS & AMS Staff: Follow the AHS Field Trip Checklist prior to submitting the Field Trip Request Form. 

• Staff submit Field Trip Request Form to Building Administrator
• Once approved by Building Administrator, submit transportation request if necessary
• Submit approved Field Trip Request Form & Student Roster to Nurse 
• Submit a Sack Lunch Request, if needed 
• Submit Overnight/ Out of State Travel Requests to School Board if necessary (at least 30 days in advance of trip) 
• Provide parents/ guardians with Field Trip Student Permission Slip
