Eligibility
To enroll in the Anacortes School District, you must reside within the 98221 zip code.
If you live outside of the Anacortes School District and would like to choice transfer in, please fill out the following forms:
Enrollment
Complete all information documents and sign electronically where indicated. Attach any court documents relating to guardianship, a parenting plan, or Limited Power of Attorney.
Certificate of Immunization (CIS)
All immunization records turned in to schools are required by state law to be medically verified. Immunization records must be turned into the school on or before the first day of attendance.
Here are some examples of medically verified immunization records:
- A CIS printed by a health care provider or school from the Washington State Immunization Information System.
- A CIS printed from MyIR which is a free Department of Health online tool that allows families to view and print their official immunization records themselves. Go to https://wa.myir.net/register to begin the sign-up process.
- A completed Certificate of Immunization Status (CIS) signed by a health care provider. Find the CIS form by visiting https://www.doh.wa.gov/SCCI and clicking on "Certificate of Immunization Status."
- A CIS filled out by you or another parent/guardian with medical records attached.
Washington State Department of Health requirements
Proof of Age
To provide proof of age for school enrollment, you can submit any of the following acceptable documents:
- Birth Certificate: A certified copy of the child’s birth certificate.
- Notarized Copy of Birth Certificate: A notarized duplicate of the birth certificate.
- Adoption Record: A adoption record that indicated child's date of birth.
- Valid Passport: A passport with the child’s date of birth.
Remember that school districts typically accept a variety of documents to verify a child’s age, ensuring a smooth enrollment process.
Special Education and Health Forms
If your student has an Individual Education Plan (IEP) or 504 Health Plan, please check the appropriate field on the enrollment form in the application process and indicate where services were provided. If you have questions please reach out to Andrea Frost afrost@asd103.org or 360.503.1218
Address Verification Documents (Must contain parent’s name and be current)
Acceptable proof of residency includes:
- Utility bills: Copies of recent utility bills. We cannot accept envelopes.
- Mortgage statements: Documents showing mortgage payments.
- Homeowners insurance documents: Proof of insurance for homeowners.
- Lease or rental agreements: These must include the first page and signature page.
- Please note that we do not accept personal correspondence or copies of envelopes
